Not just your initial training, but your annual in-service required training needs to be intense and stress inducing training. Just like the military does in Basic Training you have to do that to weed out the people who can't handle he stress of the job before they get on the job and fail when it matters.
This is a failure of standard and training, period. This person never should hve passed their academy or initial training. Had they made it past that it should have been spotted during their FTO period.
