It would be with permitting and licensing. Any business, and yes abortion providers are a business, has to have a business license. They also have to have permits to start/continue a business in various areas. If these licenses are not allowed because of the law then there would be no providers in a particular area.

In addition banks could not provide financial services to providers if they were illegal. That has been the problem with MJ dispensaries who are unable to open accounts at banks because even though MJ may be legal in the location it is still illegal at the federal level.
