Once your passport has been revoked, you must go through a process to make it valid again. You can find a description of that process at the link below:

If your State Department appeal of the revocation is not approved, you can still sue in Federal Court to have it reinstated. Apparently, Paul Manafort has either not appealed the revocation and has not taken it to court, if his appeal was denied.
His passport was revoked upon his conviction, or before his trial. The Presidential pardon does not automatically undo that revocation, apparently. Without reinstatement, it remains invalid. So, there's your answer.

I'm thinking he may have presented one of his other two passports. He apparently had 3 in 2017.

Exactly. I did small search, and tax evasion is a big reason they revoke passport.
